Preventive Health Care Beats Chronic Illness & Disease

Many serious and life-threatening diseases are preventable through changes in lifestyle and early detection. High blood pressure, high cholesterol, diabetes, and heart disease and cancers among many other illnesses are more manageable the earlier they are detected and may even sometimes be prevented. That is why preventive care is probably the most important medical care you can receive. Whatever licensed medical professional you receive it from, it is important that you follow their prescribed preventive care program, which may include risk assessments, diagnostic testing and lifestyle modifications for best results.

We recommend an annual wellness exam for all our patients, including:
  • Health risk assessment (are you at risk for heart disease? Diabetes? Other illness?)
  • Review of your medical and family history
  • List of your current providers and prescriptions
  • Chart your height, weight, blood pressure and other measurements
  • Detect any cognitive impairment
  • Develop list of risk factors and treatment options for you
  • Provide screening schedule (a checklist) for appropriate preventive care services including lab tests
  • Provide immunization to protect against infection and other diseases
  • Advanced care planning
